Parents, Complete Your Activities Registration Online!
 
Please read  and follow these steps carefully to be fully registered.  IMPORTANTOnce you complete all required steps, you are registered and can pick up your clearance form to participate in sports at the junior high or senior high level.  You must present your clearance card to your coach in order to take part in any practice activity.  This process is required prior to the first day of your sports season.  Your clearance card will be valid for all sports during the respective academic year.

Participants in all junior high and senior high sports programs must complete Steps #1-#6 unless noted below. 

High School participants in Band, cheerleading, Dance Team, FBLA, Play Production, Speech, Vocal Music, and Skiils USA must only complete Steps #2-#5.

Once you have you have completed all required steps for your activity or activities, you can turn all completed material into the Athletis office or you can scan it and submit completed material.  INCOMPLETE MATERIAL WILL NOT BE ACCEPTED, MAKE SURE THE ENTIRE PACKET IS COMPLETE.

Step 1: Athletes must have a current sports qualifying physical exam on file in the Activities Office.  Physicals are good for 1 year from exact date of last physical. It is important to note that the physical exam may not expire during the academic year the participant is registering.

Step 2:  Certify that you have read the NSAA Eligibility Poster.  Submit certification that athlete and at least one parent or legal guardian have read the NSAA Eligibility Poster.

Step 3:  Fill out and submit the Student Participation and Parent Approval Form:  Submit completed AC Student Participation and Parent Approval Form.

Step 4:  Fill out and submit the NSAA Student and Parent Consent Form:  https://nsaahome.org/textfile/spmeds/consentform.pdf.  Submit completed NSAA Student and Parent Consent Form.

Step 5:  (Students entering 9th-12th only -- all others skip to Step 6) - Fill out and submit Drug Testing Consent Form:  Submit completed Drug Testing Consent Form.

Step 6:  A parent or legal guardian of each participant must watch the video entitled "Heads Up for Concussions in Youth Sports" produced by the CDC.  The link can be found at: http://www.cdc.gov/concussion/HeadsUp/Training/index.html.  Once finished upload your certficicate of completion.  This must only be done once.  Students entering 7th grade or students who have never participated in a sport at the High School before would need to complete this requirement.  Submit certificate of completion for concussion training.
